Important information:
- Your business name as it appears on your state resale or sales tax license must be the same name used on the resale certificate form.
- We request that you provide the physical location of your business on the address line; we are unable to accept a PO box in this field
- Both the description of your business and the description of your product must be included in the required fields
- The form must be hand signed and dated.
If you choose not to provide this information within 30 days of this notice, sales tax will be charged to your orders.
